The Pennant Group, Meridian, Idaho, is seeking a Nurse Practitioner to deliver comprehensive home- and community-based health care under physician supervision. You will evaluate health history, diagnose conditions, develop care plans, prescribe medications, and coordinate with patients, families, and care teams.

The role emphasizes collaboration with primary care teams, community resources, and ongoing documentation while promoting preventive care and patient education.

Formación

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current nursing licensure in State and CPR certification.
• Master's degree with a minimum of 1 year Nurse Practitioner experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.

Responsabilidades

• Provide medical care in compliance with the state Nurse Practice Act under supervision of a collaborating physician.
• Completes history & physical examinations and documents findings in the medical record.
• Develops individualized plan of care and prescribes medications or treatments.
• Reviews clinical information and coordinates care with the primary care team.
• Educates patients and families on preventative care and medical issues.
