Job summary

Elara Caring is seeking a Hospice Nurse Practitioner to deliver high-quality home hospice and palliative care. You will work with the medical director and interdisciplinary team to manage symptoms, document care, and support families in crisis.

The role requires an NP license, Master’s in nursing, and DEA certification where applicable. You will also train clinical staff and develop guidelines to improve care quality.

Qualifications

• Master’s degree in nursing required.
• Certification through a national Nurse Practitioner licensing board.
• Current NP licensure in the practicing state.
• Active DEA certification if state allows prescription of controlled substances by APPs.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, collaborate with physicians and care team.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs to prevent avoidable hospitalizations by home care.
