Job summary

Affinity Hospice Management LLC is seeking a Music Therapist to provide therapy services to hospice patients. The role may be contracted or employed, with work addressing physical, emotional, cognitive and social needs as part of a patient-centered care plan.

The position requires a relevant degree, state certification, and CPR. A minimum of two years of experience, preferably in hospice, is expected, along with a valid driver’s license and reliable vehicle.

Qualifications

• Bachelor's or Master's degree in music therapy from an accredited program.
• State certification to practice music therapy.
• Minimum 2 years of music therapy experience; hospice experience preferred.
• Current CPR certification.
• Understanding hospice philosophy with focus on comfort.
• Licensed driver with insured vehicle meeting state requirements.

Responsibilities

• Provide music therapy to patients according to a written plan of care.
• Assess and document patient status and progress.
• Direct music therapy treatment and adjust based on goals of care.
• Educate family/caregivers and other staff on music therapy goals.
• Prepare progress summaries and report changes to the team.
• Participate in interdisciplinary group and quality improvement efforts.
• Support orientation for hospice team members as needed.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
