Job summary

Affinity Hospice Management LLC seeks a Music Therapist to provide care-driven music therapy to patients, contracted or employed, under Clinical Manager supervision. You will assess needs and implement an individualized plan of care addressing physical, emotional, cognitive and social well-being.

The role requires a degree in music therapy, state certification, CPR, and a valid driver's license; hospice experience preferred.

Qualifications

• Must hold a baccalaureate or masters degree in music therapy from an accredited program.
• State certification to practice as a music therapist.
• Minimum of 2 years of relevant music therapy experience; hospice experience preferred.
• Current CPR certification.
• Understanding of hospice philosophy and focus on comfort.
• Valid driver's license with insured vehicle.

Responsibilities

• Provide music therapy services per an individualized plan of care and evaluate progress.
• Direct music therapy treatment and tailor plans to patient needs.
• Educate families and caregivers about music therapy goals.
• Collaborate with interdisciplinary team and participate in case conferences.
• Contribute to quality improvement initiatives and orientation for new staff.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
