Job summary

Affinity Health Management is seeking a Music Therapist to provide therapeutic services addressing physical, emotional, cognitive, and social needs of patients in a hospice setting. The role may be contracted or employed, with responsibility for delivering care per individualized plans.

The candidate will work under the Clinical Manager, collaborating with nursing and therapy teams to ensure comfort-focused, patient-centered care and ongoing progress documentation.

Qualifications

• Requires a baccalaureate or master's degree in music therapy from an accredited program.
• Licensed to practice as a music therapist in the state.
• Minimum of two years of relevant music therapy experience; hospice experience preferred.
• Current CPR certification.
• Understanding hospice philosophy and comfort-focused care.
• Must have a valid driver’s license and insured vehicle.

Responsibilities

• Provide music therapy services per a written plan of care.
• Assess and document therapeutic status and progress.
• Direct music therapy treatment and prioritize needs.
• Educate families, caregivers, and staff about goals of therapy.
• Prepare and submit clinical and progress summaries.
• Collaborate with interdisciplinary team and participate in meetings.
• Contribute to quality improvement initiatives.
• Participate in hospice orientation and program activities.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
