Job summary

Affinity Hospice Management LLC is seeking a Board Certified Music Therapist (or eligible) to provide music therapy to address physical, emotional, cognitive and social needs of patients in a hospice setting. The role can be contracted or employed, reporting to the Clinical Manager.

The ideal candidate holds a music therapy degree, is licensed in the state and has CPR certification, with at least two years of experience, preferably in hospice or community settings.

Qualifications

• Bachelor's or master's degree in music therapy from an accredited program.
• Licensed to practice music therapy in the state.
• At least 2 years of music therapy experience; hospice/community preferred.
• Current CPR certification.
• Understanding of hospice philosophy and focus on comfort.
• Licensed driver with insured vehicle in good condition.

Responsibilities

• Provide music therapy per individualized care plan and assess patient status.
• Direct music therapy treatment and identify priority needs.
• Report patient responses to physician and changes in condition.
• Initiate program and train staff/family in therapy aspects.
• Prepare and submit clinical and progress summaries.
• Provide music therapy consultations to families.
• Deliver in-service education to nursing staff.
• Participate in interdisciplinary group and quality improvement activities.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
