Job summary

Affinity Health Management is seeking a Music Therapist to provide therapy services to hospice patients addressing physical, emotional, cognitive and social needs. The role may be contracted or employed, and requires directing therapy, developing plans of care, and reporting outcomes.

Candidates must hold a baccalaureate or master's in music therapy, be licensed or eligible to be licensed in the state, have CPR certification, and at least two years of experience in hospice or similar settings.

Qualifications

• Required: A bachelor’s or master's degree in music therapy.
• Certified to practice as a music therapist in the state or eligible.
• Minimum 2 years’ hospice or similar experience.
• Current CPR certification.
• Valid driver’s license.

Responsibilities

• Provides music therapy services to patients according to an individualized plan of care.
• Directs music therapy treatment and reports patient response to treatment.
• Initiates music therapy program and educates staff and family on goals of therapy.
• Prepares and submits clinical and progress summaries based on goal attainment.
• Provides music therapy consultation to families/caregivers as appropriate.
• Participates in the interdisciplinary team and quality/performance improvement activities.
• Participates in orientation for hospice team members.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
