Job summary

Affinity Health Management is seeking a Music Therapist to address physical, emotional, cognitive and social needs of hospice patients. The role may be contracted or employed and requires delivering music therapy per individualized plans.

The position reports to the Clinical Manager and involves collaboration with an interdisciplinary team, staff education, and ongoing quality improvement participation in a hospice setting in Port Saint Lucie, Florida.

Qualifications

• Bachelor's or Master's degree in Music Therapy from an accredited program.
• Certified to practice as a music therapist within the state.
• At least 2 years of appropriate experience as a music therapist; hospice experience preferred.
• Possesses and maintains current CPR certification.
• Understands hospice philosophy and focus on comfort.
• Licensed driver with a valid, insured vehicle.

Responsibilities

• Provide music therapy services to patients per a written plan of care.
• Assess and evaluate therapeutic status and contribute to the total care plan.
• Direct music therapy treatment and determine priority needs.
• Report patient responses to treatment to the physician.
• Instruct family/caregivers in music therapy goals and participate in case conferences.
• Prepare and submit clinical/progress summaries.
• Provide music therapy consultations to families as appropriate.
• Participate in orientation for hospice team members and in quality improvement activities.

JOB DESCRIPTION SUMMARY

The Music Therapist, contracted or employed, provides music therapy to address physical, emotional cognitive and social needs of patients.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

1. 1. Provides music therapy services to patients according to a written individualized plan of care. This may include, but not be limited to:
   • a. Assessing and evaluating therapeutic status, and participation in the development of the total plan of care. Evaluates home environment.
   • b. Directing music therapy treatment.
   • c. Determining priority needs for music therapy
   • d. Reporting to physician patient’s reaction to treatment or changes in condition.
2. 2. Initiates music therapy program and instructs other personnel and/or family/caregivers in certain phases of music therapy with which they may work with a patient, as well as instructing them as to the goals of the music therapy program for the patient by participating in case conferences.
3. 3. Prepares and submits clinical and progress summaries based on the attainment of goals.
4. 4. Provides music therapy consultation to families/caregivers, as appropriate.
5. 5. Provides in-service education programs for nursing personnel as needed.
6. 6. Participates in the interdisciplinary group.
7. 7. Actively participates in the quality assessment performance improvement teams and activities.
8. 8. Participates in an orientation program for hospice team members.

POSITION QUALIFICATIONS

1. 1. Possesses a baccalaureate or masters degree in music therapy approved by an accredited organization.
2. 2. Certified to practice as a music therapist within the state.
3. 3. At least (2) two years of appropriate experience as a music therapist. Community/Hospice experience is preferred.
4. 4. Possesses and maintains current CPR certification.

6. 6. Licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
