Job summary

Columbus Hospice of Georgia & Alabama seeks a Social Worker-Home Team to support patients and families facing terminal illness. The MSW role integrates with the interdisciplinary team to address psychosocial, emotional, and economic needs.

Required licensure in GA/AL and CPR/BLS certification; 2+ years hospice experience preferred. The position involves on-call rotation, home visits, and active involvement in care planning and community resources.

Qualifications

• MSW from a school accredited by CSWE.
• Must hold GA and AL social work licenses or obtain them.
• BLS/CPR certification required.

Responsibilities

• Provide psychosocial support to patients and families.
• Coordinate with interdisciplinary hospice team on care plans.
• Complete psychosocial assessments within 3 days of assignment.
• Attend team meetings and participate in care planning.
• Obtain insurance and assist with advance directives when needed.
• Maintain accurate documentation in patient records.
