Job summary

AdvisaCare Home Health and Hospice is seeking a full-time Master of Social Work (MSW) to join our hospice team serving Genesee and Lapeer Counties, with Troy as the parent agency. The MSW will perform psychosocial assessments, contribute to goal-directed care plans, and collaborate with the IDT to support patients and families.

Applicants should have an MSW from CSWE-accredited program, with hospice experience and at least one year in home health preferred.

Qualifications

• MSW degree from CSWE-accredited program.
• One year of experience in Home Health Care preferred.
• Hospice experience preferred.
• Valid Driver’s License, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and helps develop goal-directed care plans.
• Provides care under physician direction; participates in IDT conferences and notes psychosocial, emotional, spiritual, and financial conditions.
• Documents patient/family visits, calls and referrals timely and accurately in the chart.
• Conducts reassessments and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Acts as a care advocate for patient/family units and coordinates with community agencies.
• Offers ongoing counseling on death and dying; supports Bereavement Team as assigned.
• Adheres to Hospice policies and procedures.
