Job summary

VitalCaring is seeking a Master’s Social Worker (MSW) in Hospice to deliver essential emotional, psychosocial, and practical support to patients and families navigating terminal illness and end-of-life care. This field-based role works with an interdisciplinary hospice team to provide compassionate care, planning, and guidance throughout the journey.

Qualifications

• Master’s degree in Social Work (MSW)
• 1+ year of experience as an MSW in a hospice setting
• Understanding of hospice philosophy and issues related to death and dying
• Strong interpersonal skills and the ability to work tactfully with patients, families, caregivers, and community partners
• Valid driver’s license, reliable transportation, and automobile insurance when making home visits

Responsibilities

• Support patients and families by assessing psychosocial needs related to terminal illness and home care
• Provide counseling, emotional support, and guidance throughout the hospice journey
• Identify psychosocial risks and provide appropriate interventions and crisis support
• Help patients and families navigate emotional and practical challenges of end-of-life care
• Provide culturally sensitive support respecting beliefs, traditions, and family dynamics
• Educate and assist with advance directives and healthcare decision-making
• Connect with community resources and support services
• Assist with practical needs related to financial, legal, healthcare, and funeral planning
• Document and communicate with the care team and coordinate resources
• Maintain accurate clinical records and progress notes
• Collaborate with physicians, nurses, and others on the interdisciplinary team
• Ensure quality improvement and uphold standards
