Amedisys is seeking a Hospice Social Worker to support patients at home in North Carolina. This part-time role offers $27-$32 per hour plus mileage for a 30-hour work week across Alamance, Orange, Chatham, Rockingham and Caswell counties.

Qualifikationen

• Holds a masters or doctorate degree from an CSWE-accredited social work program.
• At least 1 year of social work experience in a healthcare setting.
• CPR certification current; meets state/federal requirements.

Aufgaben

• Assists patient and family with health-related financial, social, and emotional concerns.
• Provides social work services per the care plan.
• Communicates patient status changes to the healthcare team promptly.
• Educates patient and family on social work interventions and plan of care.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and coordinates interventions.
