Job summary

AMEDISYS HOME HEALTH seeks a Hospice Social Worker to support patients and families with social, emotional, and financial aspects of care. The role involves coordinating with the care team, educating families about interventions, and managing transfers and discharges as part of the plan of care.

The position is part-time (30 hours/week) with mileage reimbursement and medical benefits; territory includes Alamance, Orange, Chatham, Rockingham, and Caswell counties in North Carolina.

Qualifications

• Masters or Doctorate in social work from a CSWE-accredited program.
• At least one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assist patients and families with financial, social, and emotional concerns.
• Provide social work services per the care plan.
• Communicate changes in clinical status to the healthcare team promptly.
• Educate patients and families on social work interventions and plans.
• Perform care planning, transfer, and discharge tasks.
• Identify risks in the patient environment and implement interventions.

Attractive pay

• $27-$32 hourly + mileage

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
