Job summary

Amedisys, Inc. is seeking a part-time Hospice Social Worker (Masters level) to support patients and families at home.

You will address financial, social, and emotional needs, coordinate with the care team, and educate families on the plan of care while ensuring safety and timely communication of changes in patient status. The role requires a master's degree in social work, one year of health care social work experience, CPR certification, and compliance with state/federal requirements.

• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists patient and family with health-related financial, social and emotional concerns and effective coping skills.
• Provides social work services per the plan of care.
• Communicates changes in clinical status to the health care team promptly.
• Educates patient/family on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient environment and implements interventions to reduce risks.
• Other duties as assigned.

Attractive pay

• $27-$32 hourly + mileage
• Part Time 30 hours weekly + medical benefits

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
