Job summary

Amedisys is seeking a part-time medical social worker to support patients in their homes as part of a compassionate care team. You will address social, emotional, and financial needs, coordinate care, and help families navigate resources.

The role offers hourly pay of $32–$35 and includes benefits such as telehealth access, paid time off, and wellness programs. You’ll join a leading home health and hospice provider focused on healing patients where they are most comfortable—at home.

Qualifications

• Requires a master’s or doctorate in social work from an CSWE-accredited program.
• Minimum of one year social work experience in a healthcare setting.
• Current CPR certification required.

Responsibilities

• Assists patients and families with health-related financial, social, and emotional concerns and coping skills.
• Provides social work services in accordance with the plan of care.
• Communicates changes in the patient’s clinical status to the healthcare team promptly.
• Educates patient and family on social work interventions and treatment plans.
• Performs care planning, transfer, and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risk.

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
