Optimal Care is seeking a Hospice Medical Social Worker in Saginaw, MI. You will provide compassionate, clinical psychosocial care to patients and families facing life‑limiting illness, guiding emotional well‑being and informed decisions within an interdisciplinary hospice team.

The role requires licensure (LMSW or LCSW), CSWE‑accredited MSW/DSW, and willingness to travel to homes and facilities across service areas. Office is in Saginaw with regular weekday hours.

Qualifications

• Master’s or doctoral degree from a CSWE-accredited school of social work.
• Current state licensure as LMSW or LCSW.

Responsibilities

• Complete comprehensive psychosocial assessments.
• Provide crisis intervention and counseling.
• Support advance care planning conversations.
• Collaborate with nurses, physicians and the interdisciplinary team.
• Integrate emotional, cultural, and practical needs into the plan of care.
