Job summary

Hospice of the North Coast seeks a dedicated Social Worker (MSW) to join our compassionate team. The MSW works with patients and families under the physician's direction and participates in the interdisciplinary team to address social, emotional, and cultural aspects of terminal illness.

Must hold an MSW and have clinical experience in psychosocial care. ACSW or LCSW is preferred but not required; a valid driver’s license is needed.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• ACSW or LCSW preferred but not required.
• Clinical experience relevant to psychosocial needs of patients and families.
• Valid driver's license and auto insurance.

Responsibilities

• Conduct psychosocial assessments and evaluations.
• Provide emotional counseling to patients and families.
• Collaborate with the Interdisciplinary Team to plan and support care.
• Support the hospice philosophy and end-of-life care delivery.
