Job summary

Avera at Home Hospice – Sioux Falls is seeking a Hospice Medical Director to provide and manage medical evaluation, diagnosis, procedures, and care for patients within a recognized area of practice.

The role includes 80% as Hospice Medical Director for Avera Hospice Sioux Falls and 20% as Lead Hospice Medical Director for the Avera Hospice system. Requires MD/DO licenses and board eligibility, with strong teamwork and regulatory compliance.

Qualifications

• Medical Doctor or Doctor of Osteopathy with medical license active or eligible in the service state.
• Board certification or eligibility required for practice.
• DEA license required for medical prescribing located in the United States.

Responsibilities

• Assesses, diagnoses, and treats patients according to approved protocol and standards.
• Provides medical services and health education to patients and families.
• Performs diagnostic and therapeutic procedures as needed.
• Interprets diagnostic information and plans care accordingly.
• Maintains patient records with accurate information.
• Collaborates with care team to ensure quality patient care.
• Ensures safety and adherence to policy and regulatory standards.

Education

Medical Doctor - Board of Medicine Licensed and qualified to practice medicine in the state that services are provided and board certified or board eligible

Doctor of Osteopathy - Board of Medicine Licensed and qualified to practice medicine in the state that services are provided and board certified or board eligible and board certified or board eligible

Federal Drug Enforcement Admin - US Department of Justice Drug Enforcement Admin State and federal DEA license based on state of practice
