Job summary

Emblem Hospice is seeking a Chaplain to provide spiritual care and compassionate presence to patients and families in Chandler, AZ. You will work with an interdisciplinary team, honoring diverse beliefs to support end-of-life journeys.

Responsibilities include spiritual assessments, care planning, and education for team members, while collaborating with community clergy. Requires seminary degree and ordination; CPE preferred; valid driver’s license required.

Qualifications

  • Graduate of an accredited seminary or school of theology with a BD/MDiv or equivalent.
  • Ordained, commissioned, endorsed, or credentialed according to a recognized faith tradition.
  • Comfortable providing spiritual care in an interfaith environment for diverse beliefs and backgrounds.
  • Knowledge of hospice philosophy, end-of-life care, grief support, and bereavement services.
  • Hospice, healthcare chaplaincy, pastoral care, or related experience preferred.
  • Clinical Pastoral Education (CPE) training preferred.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to maintain accurate documentation and comply with hospice regulations.

  • Provide direct spiritual support, counseling, and comfort to patients and families.

  • Provide education and support to team members regarding spiritual care needs.

Your Impact
  • Conduct spiritual assessments and ensure appropriate spiritual care services are provided in a timely manner.
  • Participate in comprehensive patient assessments and individualized care planning to address identified spiritual needs.
  • Provide direct spiritual support, counseling, guidance, and comfort to patients and families/caregivers.
  • Collaborate with and serve as a liaison to community clergy, chaplains, and spiritual counselors.
  • Attend interdisciplinary team meetings and patient care conferences as an active member of the care team.
  • Provide education, consultation, and support to team members regarding spiritual care needs and concerns.
  • Maintain accurate and timely documentation of spiritual care services and related activities.
  • Recruit, develop, and maintain relationships with community spiritual resources to support patient and family needs.
  • Assist with the recruitment and supervision of spiritual care volunteers when assigned.
  • Participate in quality improvement initiatives and program development activities.
  • Provide funeral or memorial services upon request.
  • Coordinate periodic memorial services for families, staff, volunteers, and community clergy.
  • Perform other related duties as assigned in support of quality hospice care.
Requirements
  • Graduate of an accredited seminary or school of theology with a Bachelor of Divinity (BD), Master of Divinity (MDiv), or equivalent theological degree.
  • Ordained, commissioned, endorsed, or credentialed according to the practices of a recognized faith tradition, or possess equivalent theological training.
  • Comfortable providing spiritual care in an interfaith environment and able to respectfully support individuals from diverse backgrounds, beliefs, cultures, and traditions.
  • Knowledge of hospice philosophy, end‑of‑life care, grief support, and bereavement services.
  • Hospice, healthcare chaplaincy, pastoral care, or related experience preferred.
  • Clinical Pastoral Education (CPE) training preferred.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to maintain accurate documentation and comply with applicable hospice regulations and standards.
