Qualifications

• Current LPN/LVN license in the state of practice.
• Graduate from an accredited practical/vocational nursing program.
• Minimum 1 year acute care experience preferred.
• Current CPR/BLS certification.
• Valid driver’s license with reliable insured vehicle.
• Strong communication, clinical judgment, and ability to work independently.

Responsibilities

• Provide nursing care per the RN-developed Plan of Care.
• Monitor and document changes in patient condition; report timely to RN/physician.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within LPN/LVN scope of practice.
• Maintain accurate EMR documentation.
• Coordinate care with the home health team and participate in case conferences.
• Follow infection control, HIPAA, and company policies.

Licensed Practical/Vocational Nurse (LPN/LVN) – Hospice

Schedule: Saturday (9:00 am to 9:00 pm) and Sunday (9:00 am to 9:00 pm)

Job Summary

We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.

Responsibilities

• Provide nursing care according to the RN‑developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed.
• Assist with activities of daily living to promote patient independence.
• Educate patients and caregivers within LPN/LVN scope of practice.

This position is based on an hourly visit compensation model.

Hourly visit ranges from $35 to $40 visit commensurate with experience and depending on type of visit.
