Job summary

Lorian Health in Las Vegas, NV is seeking an experienced LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.

You will document changes, educate patients and caregivers, and maintain EMR records while following HIPAA and infection-control standards.

Qualifications

• Current LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.

Licensed Practical/Vocational Nurse (LPN/LVN) – Home Health

Job Summary

We’re looking for a skilled and compassionate LPN/LVN to provide in‑home nursing care under the supervision of a Registered Nurse (RN). You’ll support patients with daily care needs, monitor their condition, and help ensure safe, high‑quality home health services.

Responsibilities

• Provide nursing care according to the RN‑developed Plan of Care.
• Monitor and document changes in patient condition; report to RN/physician as needed.

• Educate patients and caregivers within LPN/LVN scope of practice.

Requirements

• Current LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Minimum 1 year acute care experience preferred.
• Current CPR/BLS certification.
• Valid driver’s license, reliable insured vehicle.
• Strong communication, clinical judgment, and ability to work independently.

Physical Requirements

• Ability to lift/transfer patients and perform physical caregiving tasks.
• Ability to pass required physical exams and annual PPD testing.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $35 to $40 visit commensurate with experience and depending on type of visit.
