Job summary

Providence at Home with Compassus in Portland West, OR is seeking a Home Health LPN/LVN. You will administer skilled nursing care to patients in their homes under the supervision of the RN Case Manager, weekdays, day shift, full-time.

You will follow physician orders, educate patients and families, maintain records, and participate in care conferences. Requires LPN license; two years preferred; hospice/home health experience preferred.

Qualifications

• Certificate of an accredited Practical or Vocational Nursing program (state approved).
• Two years of experience as an LPN in a clinical care setting preferred.
• Hospice/home health experience preferred.

Responsibilities

• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Educates patient and family regarding care needs.
• Maintains accurate and complete patient records and reports changes to the RN Case Manager.
