Qualifications
1 year LVN/LPN experience in a clinical care setting.
Current LVN/LPN license in good standing.
Current CPR certificate.
Must read, write and comprehend English.
Responsibilities
Provide nursing care under RN supervision in clients' homes.
Coordinate care with the interdisciplinary team and families.
Assist in planning and implementing the Plan of Care with the RN.
Complete documentation timely per policy and standards.
Maintain infection control practices and OSHA guidelines.
