Job summary

Ohio’s Hospice, Inc. in Dayton, OH is seeking an experienced Hospice LPN Team Nurse to deliver direct patient care with the interdisciplinary team.

The role involves traveling to patients in facilities and homes within Eaton/Preble County and SW Montgomery counties, with a Monday–Friday schedule and weekend rotation. CPR certification and Ohio licensure are required, plus driving ability and clean background/drug checks.

Qualifications

• LPN with the State of Ohio license and no board actions.
• Minimum one year of nursing experience preferred.
• CPR certification required.
• Computer skills to document services and care.
• Valid driver’s license with a safe driving record.
• Must pass a criminal background check.
• Must pass a 10-panel drug screen.

Responsibilities

• Provide direct hospice nursing care to patients.
• Collaborate with interdisciplinary team to support families.
• Travel to patient locations in facilities and homes across the service area.

Hospice LPN – Making every moment matter

As a Hospice LPN, you will play a vital role in enhancing quality of life for patients and supporting families during some of their most important moments. Working alongside a dedicated interdisciplinary team, you'll deliver compassionate care that brings comfort, dignity, and peace.

This is a FT position, M-F 8am-5pm with every 6th weekend rotation and 1 holiday requirement per year. This is a driving position seeing patients in both facilities and homes within the greater Eaton/Preble County and SW Montgomery counties

Qualifications of this role:

• LPN with the State of Ohio without any board actions.
• One year of nursing experience is preferred.
• CPR Certified.
• Computer skills are sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10-panel drug screen
