Job summary

Ohio’s Hospice is seeking a Hospice LPN Team Nurse to provide direct patient care in Dayton, OH, supporting families during pivotal moments in their care journey.

This full-time role features a Monday–Friday 8am–5pm schedule with every 6th weekend rotation and one holiday per year, including travel to facilities and patient homes in Eaton/Preble County and SW Montgomery counties. The team emphasizes comfort, dignity, and peace for patients and families.

Qualifications

• LPN license in the State of Ohio is required.
• One year of nursing experience is preferred.
• CPR certification is required.
• Computer skills sufficient to document services and care.
• Ability to drive during daytime, nighttime or inclement weather.

Responsibilities

• Provide direct patient care to hospice patients in homes and facilities.
• Collaborate with an interdisciplinary team to develop and adjust care plans.
• Document patient information accurately and promptly.
• Ensure safety, dignity, and comfort for patients and families.

Hospice LPN – Making every moment matter

As a Hospice LPN, you will play a vital role in enhancing quality of life for patients and supporting families during some of their most important moments. Working alongside a dedicated interdisciplinary team, you'll deliver compassionate care that brings comfort, dignity, and peace.

This is a FT position, M-F 8am-5pm with every 6th weekend rotation and 1 holiday requirement per year. This is a driving position seeing patients in both facilities and homes within the greater Eaton/Preble County and SW Montgomery counties

Qualifications of this role:

• LPN with the State of Ohio without any board actions.
• One year of nursing experience is preferred.
• CPR Certified.
• Computer skills are sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10-panel drug screen
