Job summary

Ohio’s Hospice is seeking a dedicated Hospice LPN to provide compassionate, 1:1 bedside care for patients in homes or facilities within the Dayton area. This PT Day shift role (8am-8pm) requires driving to patient locations and delivering care under RN supervision while documenting accurately in our care systems.

Qualifications

• Licensed LPN in Ohio with current license and no board actions.
• At least 1 year nursing experience preferred.
• CPR certified.
• Able to drive to patient locations and handle inclement weather.
• Valid driver's license with clean record and automobile insurance.
• Pass background check and 10-panel drug screen.
• Proficient in documenting care using computer systems.

Responsibilities

• Provide 1:1 bedside care for patients actively declining at home or in facilities.
• Work under RN supervision in home settings.
• Drive to patient locations during daytime, evenings, or adverse weather.
• Document services and care delivered using the electronic records system.

Job description

Hospice Individualized Care LPN - Making every moment matter

• This is a PT Day shift position 8am-8pm and will require DRIVING to patients within the Dayton/Springfield area
• This position will provide 1:1 with the patient at the bedside for patients that are actively declining

As a Hospice Individualized LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.

Qualifications for the Hospice LPN:

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR Certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen
