Job summary

Sanford Health in Luverne, MN is seeking a compassionate Licensed Practical Nurse (LPN) or Med Aide (MA) to join the cottage hospice team. You will provide comfort-focused, patient-centered care to individuals and families during one of life's meaningful stages.

This full-time home care role features a 36-hour work week, night shifts, and a 1:4 patient ratio. You will collaborate with an interdisciplinary team to manage symptoms, support emotional well-being, and maintain safety and dignity for

Qualifications

Graduate from an accredited practical nursing program and hold an LPN license.
Current unencumbered LPN license; multi-state licensure may be required depending on location.
BLS for healthcare providers required.
One year of professional nursing experience preferred.
Possession of a valid driver’s license may be required depending on location.
IV therapy/management skills may be required in North Dakota per state requirements.

Responsibilities

Provide comfort-focused, patient-centered care to home care patients.
Collaborate with an interdisciplinary team to manage symptoms and ensure patient comfort and safety.
Administer treatments and medications as directed.
Demonstrate nursing competencies within the home care setting and adapt care to age-specific needs.
