Job summary

Mountain Valley Hospice in Pilot Mountain, NC, is seeking a Licensed Practical Nurse (LPN) to join our team, delivering compassionate care in patients' homes, SNFs, and ALFs. You will provide direct patient care, work with an interdisciplinary team, and help monitor patient conditions to support optimal comfort and quality of life.

Qualifications

• Valid Licensed Practical Nurse License.
• Exceptional interpersonal and communication skills, with a compassionate and patient-centered approach.
• Ability to work both independently and as part of a team, with excellent time management skills.

Responsibilities

• Deliver direct patient care to patients, including administration of medication, wound care, treatments, and procedures under the supervision of a registered nurse.
• Collaborate with the interdisciplinary team to address patient and family needs, including connections with aides, social workers, chaplains, and other providers.
• Monitor and document changes in patient conditions and communicate these effectively to the care team.
