Job summary

MJHS Hospice & Palliative Care in Delaware is seeking an LPN to visit hospice patients at home or in nursing homes, documenting each visit in the EMR and reporting changes to the RN. The schedule includes backup on-call 2–3 times per month and 2–3 holidays per year.

Graduate from an accredited LPN program with at least one year of Med/Surg experience and one year of Hospice or Oncology nursing. A valid driver's license and good PC skills are required; visits cover all locations except Manhattan.

Qualifications

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum of one year as an LPN in Med/Surg nursing and one year Hospice or Oncology Nursing experience.

Responsibilities

• Provide visits to hospice patients in their home or in a Nursing home.
• Document the visit in the Electronic Medical Record (EMR).
• Report any changes in patient condition to the Registered Nurse.
• Participate in backup On Call capacity 2–3 times per month and 2–3 holidays per year.
