MJHS Hospice invites a dedicated Licensed Practical Nurse to provide home and nursing home visits, documenting care in the EMR and communicating changes to the RN. The role includes 2-3 on-call shifts per month and 2-3 holidays annually, requiring compassionate end-of-life support.

Applicants must graduate from an accredited LPN program with at least one year of Med/Surg experience and one year in Hospice or Oncology nursing, plus strong PC skills and a valid driver's license for all visits

Formación

• Graduate from an accredited Licensed Practical Nurse Program
• Must have a minimum of one year experience as an LPN in Med/surg nursing and one year of Hospice or Oncology Nursing experience
• Good PC skills a must

Responsabilidades

• Provide visits to hospice patients in their home or in the Nursing home
• Document the visit in the Electronic Medical Record (EMR) and report changes to the RN
• Participate in backup On Call capacity 2-3 times per month and 2-3 holidays per year
