MJHS Hospice is seeking a dedicated Licensed Practical Nurse to join our home-visit care team in New Jersey. You will provide visits to hospice patients at home or in nursing facilities, document in the EMR, and communicate changes to the RN.

The schedule includes on-call 2-3 times per month and 2-3 holidays per year; Manhattan visits are excluded. Must have 1 year Med/Surg LPN experience and hospice or oncology experience; strong PC skills and a valid driver's license are required.

Qualifications

• Graduate from an accredited LPN program.
• Minimum 1 year as an LPN in med/surg nursing.
• At least 1 year Hospice or Oncology nursing experience.
• Good PC skills required.

Responsabilités

• Provide visits to hospice patients at home or in the Nursing home.
• Document visits in the EMR and report changes to the RN.
• Schedule includes 2-3 on-call visits per month and 2-3 holidays per year.
