Hospice LPN — Home Visits & On-Call Support

Job summary

MJHS Hospice is seeking a licensed practical nurse to provide compassionate patient care in home or nursing facility settings in New Jersey. You will perform patient visits, document care in the EMR, and communicate changes to the RN to ensure continuity of care.

The role includes a backup On Call schedule (2-3 times per month) and 2-3 holidays per year, requiring reliability, PC skills, and a valid NJ LPN license. Travel to Manhattan is excluded, with driving required for all visits.

Qualifications

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum of one year experience as an LPN in Med/surg nursing.
• One year of Hospice or Oncology Nursing experience.

Responsibilities

• Provide visits to hospice patients in home or Nursing home.
• Document visits in the Electronic Medical Record and report changes to the RN.
• Participate in backup On Call 2-3 times per month and 2-3 holidays per year.
