MJHS seeks an LPN to conduct home and nursing home hospice visits, documenting patient status in the EMR and communicating changes to the supervising RN. The role includes on-call coverage 2–3 times monthly and 2–3 holidays per year.

Applicants must be a graduate of an accredited LPN program with at least 1 year of Med/Surg nursing and Hospice/Oncology experience, plus solid PC skills and a valid driver’s license.

Formación

• Graduate from an accredited Licensed Practical Nurse Program.
• Minimum 1 year experience as an LPN in Med/Surg nursing and 1 year Hospice or Oncology Nursing experience.
• Good PC skills are required.
• Driver's license and reliable transportation required for all visits (Manhattan excluded).

Responsabilidades

• Provide visits to hospice patients in their home or Nursing home.
• Document each visit in the Electronic Medical Record and report changes to the RN.
• Participate in backup On Call 2–3 times per month and 2–3 holidays per year.
