Job summary

EvergreenHealth Hospice seeks a licensed practical nurse to provide in-home nursing services under RN supervision. You will deliver direct patient care, coordinate education for patients and families, and continuously assess and adjust care plans with physicians and the interdisciplinary team.

Strong communication, reliability, and a current WA LPN license are essential, along with a driver’s license and BLS certification. Driving to patient homes is required.

Qualifications

• Licensed practical nurse (LPN) with WA state license is required.
• Current Washington State Driver’s license and proof of insurability.
• Current Healthcare Provider BLS certification by date of hire.
• Reliable transportation.

Responsibilities

• Provide direct patient care in the patient’s home.
• Coordinate patient and family education.
• Plan, implement, and evaluate patient care with the team.
