Job summary

Bgcarenav is seeking a Licensed Practical Nurse (LPN) to provide skilled hospice care in patients' homes and other settings within Fayette County and Jessamine County. Working under the supervision of a Registered Nurse, you will deliver individualized end-of-life care in alignment with the plan of care to promote comfort, safety, and dignity.

This role includes performing nursing tasks, monitoring changes in patient condition, educating patients and caregivers, and documenting care in the EMR.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Current LPN licensure in good standing with the Kentucky Board of Nursing.
• Minimum of two years of nursing experience; hospice/medical-surgical/long-term care preferred.
• Demonstrates compliance with professional nursing standards and hospice philosophy.
• Must be a licensed driver with an automobile in good working order and insured.

Responsibilities

• Provide skilled and compassionate nursing care to hospice patients in their place of residence.
• Work under the supervision of a Registered Nurse and follow the plan of care.
• Deliver care to promote comfort, safety, and quality of life at end of life.
• Monitor and report changes in patient condition; educate patients and caregivers.
• Document care accurately and timely within the EMR.
