Job summary

Anew Hospice is seeking a licensed practical nurse to provide direct patient care under RN supervision, delivering end-of-life comfort and coordinating with the interdisciplinary team.

This on-site role covers Lafayette and Kokomo, IN, with competitive wages, mileage reimbursement, and a full benefits package including medical/dental/vision, 401k, and PTO. Monday–Friday daytime shifts preferred.

Qualifications

• Graduate of an accredited practical nurse program.
• Three (3) years nursing experience.
• Hospice or palliative care experience preferred.

Responsibilities

• Provide direct patient care under RN supervision.
• Follow the plan of care and administer treatments.
• Document patient services accurately and timely.
• Educate patients and families about diagnosis and care.
• Coordinate hospice services with the interdisciplinary team.
• Adhere to PPE and HIPAA requirements.
• Demonstrate CARE values and maintain attendance.
