Job summary

Guardian Angel Hospice in Kokomo, IN seeks a compassionate LPN to support end-of-life care under physician and IDT guidance. You will deliver specialized nursing services, educate patients and families, and document care while safeguarding confidentiality.

The role emphasizes collaborative planning, aseptic technique, and timely communication with the care team. You should be reliable, possess a valid driver’s license, and be able to adapt care plans to diverse settings while promoting comfort

Qualifications

• Reliable transportation and a valid driver’s license.
• Ability to make sound nursing judgments in diverse field settings.
• Proficiency in assessing patient needs and creating tailored care plans.
• Strong scheduling and management capabilities.

Responsibilities

• Deliver services in compliance with organizational policies, procedures, and regulations defining the scope of practice.
• Prepare clinical and progress notes detailing the specialized nursing services provided.
• Assist the RN and/or physician with end-of-life care duties.
• Support the RN in implementing the plan of care.
• Prepare equipment and materials for treatment adhering to aseptic techniques.
• Help patients learn appropriate self-care techniques.
• Assess patient and family responses to care.
• Ensure relevant information is communicated to the team.
• Submit documentation according to agency policies.
• Attend in-service programs and interdisciplinary team (IDT) meetings.
