Job summary

Allied Services is seeking a Licensed Practical Nurse to provide direct nursing care within a Hospice IDT setting. You will support patient and family needs, coordinate with the care team, and contribute to comfort-focused plans.

The role requires an accredited LPN program graduate with at least two years of nursing experience; hospice experience preferred and CPR certification is required.

Qualifications

• Graduate of an accredited LPN program is required.
• Minimum two years general nursing experience; hospice experience preferred.
• CPR certification must be obtained and maintained.

Responsibilities

• Provide direct nursing care under supervision of Clinical Director/ RN.
• Identify patient/family needs and coordinate care with the IDT.
• Administer medications, chart treatments, and monitor patient status.
• Collaborate with the interdisciplinary team to support Hospice care.
• Maintain accurate documentation and ensure patient safety.
• Assist in end-of-life care planning and family education.
