Job summary

Empath Health, partner Tidewell Hospice, is seeking a licensed practical nurse to provide bedside observations, prescribed care, and emotional support to patients with advanced illness. Under the RN case manager, you will report changes and help ensure patient comfort per the plan of care.

Qualified candidates hold a Florida LPN license, graduate from an approved nursing program, and bring at least one year of acute care experience; hospice LPN experience is preferred.

Qualifications

• Graduate of an approved nursing program.
• Must hold a Florida LPN license.
• Minimum one year of experience in an acute care setting; hospice LPN experience preferred.

Responsibilities

• Provide continuous bed-side clinical observations and prescribed care.
• Report significant changes in patient condition and family needs to the RN and care team.
• Collaborate with the RN to process physician orders to maximize patient comfort.
• Assist with triage and care centers overnight when Continuous Care cases are pending.
