Job summary

Tidewell Hospice, part of Empath Health, is seeking a compassionate LPN to provide continuous bedside nursing support in southwest Florida. You will work under the RN case manager, delivering prescribed care, monitoring patients, and reporting changes to ensure comfort and dignity at the end of life.

The role requires an active Florida LPN license and at least one year in acute care, with hospice experience preferred.

Qualifications

• Graduate of an approved nursing program.
• Current Florida LPN license.
• Minimum one year experience in an acute care setting.
• Previous hospice LPN experience preferred.

Responsibilities

• Under the direction of the RN case manager, provide continuous bedside clinical observations, prescribed care, physical and emotional support.
• Report significant changes in patient condition and family needs as needed.
• Accept and process physician orders with the RN to maximize patient comfort.
• Assist triage and care centers overnight when Continuous Care cases are pending.
