Job summary

VitalCaring is seeking a dedicated Licensed Practical Nurse (LPN/LVN) to join our hospice team and provide compassionate, patient-centered end-of-life care. You will work closely with RNs, aides, social workers, and chaplains to tailor comfort-focused plans and ensure dignity at every visit.

Key responsibilities include direct patient care, symptom management, care plan implementation, and education for patients and families.

Qualifications

• Active LPN/LVN license in the state of practice.
• Current CPR certification.
• 1–3+ years of nursing experience preferred (hospice, home health, or med-surg).
• Strong clinical judgment, communication skills, and emotional intelligence.
• A genuine passion for patient-centered, end-of-life care.

Responsibilities

• Provide direct patient care in accordance with the Nurse Practice Act, with a focus on end-of-life support.
• Implement individualized plans of care under the supervision of a Registered Nurse.
• Deliver effective pain and symptom management, monitoring and adjusting based on patient response.
• Support patients with daily care needs while ensuring comfort, dignity, and safety.
• Educate patients and families on disease progression, medications, and comfort measures.
