Job summary

Pathways Healthcare, Llc is seeking a compassionate Licensed Practical Nurse (LPN) to join our growing home health team. You will deliver direct patient care in patients' homes while collaborating with RNs, physicians, patients, and families to achieve high-quality outcomes.

Qualifications

LPN license in Massachusetts active status.
Graduate of an accredited practical nursing program.
Experience in home health, hospice, or geriatrics preferred.
Proficient with EMR systems.
Excellent communication and interpersonal skills.
Current BLS CPR certification.
Valid driver’s license and insured vehicle.

Responsibilities

Provide skilled nursing care under RN supervision and plan of care.
Monitor and document patient condition, vital signs and responses to treatment.
Administer medications and treatments as prescribed.
Assist with implementation and evaluation of patient care plans.
Educate patients and caregivers on medications, treatments, and disease management.
Communicate changes in patient condition to supervising RN and physician.
