Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our home health team in Ohio. You will follow patient care plans, monitor progress, and liaise with physicians and families to ensure coordinated care.

The ideal candidate has a current LVN/LPN license, a nursing credential from an accredited program, and at least 1 year of clinical experience. Travel between patient homes is required with a valid driver's license.

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational / Practical Nurse.
• 1 year of experience in a clinical care setting.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
