Job summary

Elara Caring in Purcell, OK seeks a Licensed Vocational / Practical Nurse to join our home health team, delivering compassionate, high-quality care in patients' homes. You will follow care plans, monitor progress, educate families, coordinate with the RN Case Manager, and document medications according to guidelines.

A valid LVN/LPN license and at least one year in a clinical setting are required; hospice/home health experience is preferred.

Qualifications

Current State LVN/LPN license.
Graduate from accredited nursing program.
At least 1 year in a clinical care setting.
Ability to lift and transfer up to 50 pounds.
Valid driver’s license or reliable transportation.
Home health or hospice experience preferred.

Responsibilities

Follow each patient’s individualized care plan and physician orders.
Monitor patient progress and report changes or safety concerns to the care team.
Help patients and caregivers learn self-care techniques to support their health.
Work with the RN Case Manager to update care plans as patient needs change.
Document medication regimens and update medication profiles according to agency guidelines.
Communicate with physicians, patients, families, and the care team to ensure coordinated care.
Help prevent avoidable hospitalizations and ER visits through proactive patient care.
Deliver compassionate, high-quality care that supports positive patient outcomes.
