Job summary

Elara Caring in Columbus, Ohio is seeking a Licensed Vocational Nurse (LVN) to join our home health team. You will follow care plans and coordinate with physicians to deliver compassionate care in patients' homes.

Active LVN license, 1 year of clinical experience, ability to lift up to 50 pounds, and reliable transportation for home visits are required; hospice/home health experience is preferred.

Qualifications

• Licensed Vocational Nurse (LVN) in good standing.
• Graduate of an accredited nursing program (Certificate/Diploma/Associate).
• Minimum 1 year of clinical care experience.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as needed.
• Document medication regimens and update medication profiles.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive care.
• Deliver compassionate, high-quality care to support positive patient outcomes.
