Job summary

Elara Caring is hiring a Licensed Vocational / Practical Nurse to provide in-home care, following individualized care plans and physician orders. You will monitor progress, educate patients and families, and coordinate with the RN Case Manager to adapt plans as needs change. Strong documentation and communication with the care team are essential.

Experience in home health or hospice is preferred. A valid state license and 1 year of clinical care experience are required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques.
• Update care plans with the RN Case Manager as patient needs change.
• Document medication regimens and update medication profiles.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
