Bristol Hospice-eugene Llc in Oregon seeks a compassionate Licensed Practical Nurse (LPN/LVN) to provide direct end-of-life care, implement care plans, and document services. The role requires current state licensure and at least three years of nursing experience, with hospice knowledge highly preferred.

Expect collaborative teamwork with therapists and social workers. The position is Monday–Friday, 8am–5pm, serving Linn and Benton Counties.

Qualifications

• Must currently be licensed as an LPN/LVN in the State.
• Three years nursing experience preferred.
• Hospice or end-of-life care experience preferred.
• Understanding of hospice philosophy and patient-centered care.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing plan of care with assessment.
• Document patient services accurately and timely.
• Educate patient and family/caregivers on diagnosis and self-care.
• Coordinate hospice services with interdisciplinary team; report needs.
