Job summary

Bristol Hospice is seeking an LPN for full-time day shift in Eugene, OR. Join our team to deliver compassionate end-of-life care and support to patients and families in Linn and Benton Counties.

You will perform direct nursing duties, document care, educate families, and collaborate with the interdisciplinary team to ensure high-quality hospice services.

Qualifications

• Must currently be licensed as an LPN/LVN in the state.
• Three years nursing experience; hospice or medical/surgical preferred.
• Understand hospice philosophy and needs of terminally ill patients.
• Flexible in work hours and shifts.
• Maintain comprehensive knowledge of information systems and software used in care.

Responsibilities

• Provide direct patient care as part of end-of-life care.
• Document services accurately to reflect the plan of care.
• Educate patients and families about diagnosis and care.
• Coordinate hospice services with interdisciplinary team.
• Manage pain and symptoms effectively.

Schedule: Monday-Friday, 8am-5pm

Coverage Area: Linn & Benton Counties

Qualifications: An Average Day:

• Provide direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group; examples include hospice aide, OT, PT, SW, ST, Dietician or Clinical Supervisor
• Use and prepare equipment and supplies effectively, adhering to aseptic technique
• Provide appropriate pain/symptom management
• Evaluate patients response to treatments/medications
• Participate in personal, professional growth and development
• Participate in organization's quality assessment performance improvement teams and activities
• Perform other duties as assigned by the registered nurse
• All other duties as assigned

Requirements:

• Must currently be licensed as an LPN/LVN in the State
• Must have three (3) years nursing experience, community health/hospice or medical/surgical experience is preferred
• Must understand the philosophy of hospice concept and understand needs of the terminally ill
• Must be flexible in work hours
• Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
