Job summary

Capital City Hospice in Columbus, OH is seeking an experienced LPN to join a dedicated hospice team. You will provide direct patient care, perform assessments, document services, and educate patients and families about diagnoses and care plans.

Join a supportive team with low caseloads, flexible scheduling, and a strong commitment to compassionate care for Central Ohio residents.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Current LPN license in the state.
• At least 1 year of medical/surgical experience (home health or hospice preferred).
• Current CPR certification.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act.
• Complete comprehensive assessments and follow the individualized plan of care.
• Document patient services accurately and in a timely manner.
• Educate patients and families/caregivers about diagnoses and care plans.
• Collaborate with the interdisciplinary team (aides, OT, PT, MSW, Dietitian, Clinical Supervisor).
• Manage pain and symptoms effectively and evaluate patient response.
• Support patients and families with compassion, professionalism, and respect.
