Job summary

Capital City Hospice in Columbus, Ohio, seeks a dedicated LPN to join their team focused on delivering compassionate hospice care. The ideal candidate will have at least three years of nursing experience, preferably in hospice or community health settings, and be committed to patient education and symptom management.

Compensation ranges from $28 to $34 per hour, and benefits include a 401(k) plan with matching, mileage reimbursement, and employee discounts.

Qualifications

  • Must have three years nursing experience, with a preference for community health/hospice or medical/surgical experience.
  • Maintains a current CPR certification.
  • Licensed driver with an insured and operational automobile.

Responsibilities

  • Provides direct patient care as defined in State Nurse Practice Act.
  • Implements current nursing practice following a comprehensive assessment and care plan.
  • Documents patient services accurately and timely.
  • Coordinates hospice services, reporting needs to the interdisciplinary group.
  • Evaluates patient’s response to treatments and medications.

What You’ll Do
  • Provides direct patient care as defined in State Nurse Practice Act
  • Implements current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care
  • Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical Supervisor
  • Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications
Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program

  • Currently licensed as an LPN/LVN in the practicing state
  • Maintains a current CPR certification.
